The sugar, and the missing fiber

What it rests onTwo papers from the SUCRE cohort. Obesity 2016 (43 children, dietary sugar from 28% to 10%, calories adjusted daily for weight maintenance, results controlled for weight change by ANCOVA) and Gastroenterology 2017 (41 children with habitual fructose intake above 50 g/day, liver fat by magnetic resonance spectroscopy, de novo lipogenesis by stable isotope tracers, NCT01200043). His mechanism: fructose is metabolised in the liver in a way glucose is not, and converting it to fat is a modifiable pathway.

Not the sugar, at least not for how much you eat, and probably not identifiable yet

What it rests onCell Metabolism 2019, 20 adults, two weeks per arm, ad libitum intake, every calorie presented and every calorie left weighed. Separately, Nature Medicine 2021 found people ate 689 kcal/day MORE on an animal-based ketogenic diet than on a plant-based low-fat one, which is the opposite of what a carbohydrate-insulin mechanism predicts.

Overview

The two sides are not measuring the same thing, and that is the most important thing to know here. Lustig's outcome is metabolic biomarkers in children who already have metabolic syndrome, eating a provided diet, with calories held constant by design. Hall's outcome is how much healthy adults spontaneously choose to eat when food is put in front of them.

Cutting sugar could improve a liver measurement without changing how much anyone eats, and ultra-processed food could drive overeating without sugar being the reason. Both can be true at once, and a user reading only the headlines would think they cannot.

Where they agree

Total, on the action, and it should be shown before anything else. Lustig reviewed Hall's trial in print and concluded 'this study shows that real food works, and processed food doesn't, take it to the bank.' Hall's own conclusion says the recommendation 'could potentially be embraced across a wide variety of healthy dietary approaches'.

Both also agree the mechanism is unresolved: Lustig lists six live candidates in the same review (fiber, energy density, carbohydrate, insulin and leptin, the microbiome, satiety) and says the trial cannot separate them. Neither has any commercial interest in this particular question.

What would settle it

A trial that varies added sugar alone, isocalorically, in free-living adults without an existing diagnosis, with both metabolic biomarkers and ad libitum intake measured. Lustig's own trials have no control arm and last nine days; Hall's have a control arm and do not vary sugar independently. Each side's evidence is exactly what the other's design cannot produce, which is why this has not resolved in fifteen years.
