Overview

Campbell is arguing about long-term chronic disease incidence in whole populations and his evidence base is a rural Chinese cohort eating about one-tenth the American intake of animal protein, plus rodent carcinogenesis.

Teicholz is arguing about the evidence for a population-wide restriction policy in Western countries, and her trial evidence is largely in adults with or at risk of cardiometabolic disease. They are not describing the same population and they are not asking the same question: his is "what causes this", hers is "what did the trials that tested the advice actually show".

Where they agree

More than either camp usually admits, which is what makes this one worth reading. Both hold that saturated fat is not the primary cause of heart disease. Both hold that the fifty-year focus on it was an error.

Both reject single-nutrient reasoning in favour of whole foods, and both can cite the same sentence from the Astrup paper approvingly: that health effects "cannot be predicted by the content of any individual nutrient". Campbell quotes it and then says the authors do not follow it. The disagreement starts entirely after that shared premise, which makes it a clean demonstration that agreeing on the evidence does not settle what to eat.

What would settle it

Campbell says his conclusion rests on convergence across four lines, so a single trial would not move him; what would is a demonstration that the animal-protein effect he found in rodents does not generalise, or a population with high animal-food intake and low chronic disease that survives adjustment.

Teicholz's stated standard is randomised trial evidence with hard outcomes, so a long-duration trial of animal-food restriction with mortality endpoints would move her, and its absence is her current argument.
