Did the twin study show that going vegan is better, or just that eating less is?

The vegan arm improved LDL, fasting insulin and weight against a genetically identical control, and the calorie difference is disclosed rather than hidden.

What it rests on22 identical twin pairs randomised within pair for 8 weeks, meals delivered for the first four weeks. LDL down 13.9 mg/dL, fasting insulin down 2.9 uIU/mL and weight down 1.9 kg relative to the omnivorous co-twin. The paper's own limitations section states the study was not designed to be isocaloric and that the LDL change cannot be separated from weight loss, alongside the small sample, the eight-week duration, the healthy baseline population and the absence of follow-up. His own summary of the result is deliberately weaker than the documentary's: what matters more than going strictly vegan is including more plant foods.

The study fails a basic requirement of hypothesis testing, so it cannot support the claim made for it.

What it rests onA categorical failure to isolate and test a specific independent variable: the vegan arm ate roughly 200 fewer calories a day (1628 against 1815 kcal/day, about 11 percent) and far less saturated fat, so at least three variables moved at once. He argues eight weeks is too short to permit proper acclimation to veganism or to surface deficiencies that take months or years, and notes the vegan twins reported lower enjoyment of the food. His conclusion is that the trial demonstrates calorie restriction and healthy diet composition can improve metabolic health, not that plant-exclusivity did it.

Overview

22 twin pairs, 77 percent female, mean age 39.6, mean BMI 25.9, already healthy, with LDL starting inside the normal range at 110.7 mg/dL. Both men are talking about the same 44 people, which is rare and makes this an unusually clean disagreement: there is no population mismatch to hide behind, only an interpretive one.

Where they agree

Both agree the measured changes happened. Both agree the arms differed in calories and in saturated fat. Both agree eight weeks is short. Gardner's paper concedes the isocaloric point in print, which means the dispute is over what the finding licenses, not over what the finding is.

What would settle it

An isocaloric version of the same twin design, matched on saturated fat, running long enough to show whether adherence and enjoyment hold. Both sides have effectively asked for the same next study, which is the most useful place a disagreement like this can land.
